Nottingham University Hospitals NHS Trust said that on 18 August 2026 staff running routine technical work reused pre-written instructions meant for a radiotherapy system and missed a setting, so the maternity database covering September 2011 to November 2022 was overwritten. The trust says clinical information, including notes, observations and test results, is still available. It could not fully restore the access history that records who viewed those maternity records. The trust reported the loss to the Information Commissioner's Office and to Nottinghamshire Police, and says current maternity care was not affected.
